Every three years, the Ventura County Community Health Improvement Collaborative  – made up of local hospitals, health clinics, Ventura County Public Health, Ventura County Behavioral Health, managed care plans, and many other healthcare partners – work together to lead a comprehensive, county-wide community health needs assessment.  This survey is a crucial first step in bringing our community together in 2025 to align priorities to meet local needs in the years to come.

Responses will be collected through Feb. 28.

These responses will provide an overview of pressing health concerns and challenges across  Ventura County.  To make it as effective as possible, it’s crucial that a large, diverse, representative pool of resident feedback is collected, so please help by providing your feedback.
