Aug 2016: Health District Named Allied Health Professional of the YearCamarillo Health Care District Receives Award for Outstanding Partnerships and Innovation
Camarillo, CA – The Camarillo Health Care District (District) is honored to announce it has been named Allied Health Professional of the Year by the Ventura County Medical Resource Foundation at the David Fainer, MD & Leo Tauber, MD Award ceremony. The Fainer and Tauber award is given for outstanding contributions by those involved in delivery of health care in Ventura County.
The District was recognized for exceptional efforts in the following areas:
- Transitioning patients from hospital to home through the District’s Care Transitions program, which seeks to maximize the return to health at home, empower patients to manage their health conditions at home, and support hospitals, physician groups, payor groups and other providers in reducing avoidable readmissions.
- Developing innovative solutions and partnerships to support caregivers including developing and facilitating a six-week “Tools for Caregiving” course, and implementing the Rosalynn Carter Institute REACH Program for Alzheimer’s caregivers.
- Empowering and improving overall community health by facilitating a wide range of programs including evidence-based screenings and services addressing health issues such as diabetes, pain management, memory impairment, and fall prevention; caregiver education, training and support; health screenings; advocacy programs; and a variety of support groups, body strengthening and fitness classes, nutrition and relaxation.
District CEO Kara Ralston said she holds this award as a high honor. “We have an incredible team of licensed, certified and trained professionals who are dedicated to ensuring that we respond appropriately and effectively to support and improve the health of our community through a variety of partnerships and programs,” Ralston said. “Receiving this award affirms the District’s commitment to the triple aim philosophy of improving the patient experience of care, improving the health of populations; and reducing the per capita cost of health care. This award is truly a great honor.”
The award was received at the 23rd Annual David Fainer, MD & Leo Tauber, MD Awards Ceremony on August 18th at the Lloyd-Butler Estate in Oxnard.
